Parental Directive Form-Opt Out of State Assessments
Use the link below to access the parental directive form. The form may be used to opt of the ND State Assessments (Language Arts, Math, and Science), as well as ND Alternate Assessments and local interim assessments. A separate form must be filled out for each student and for each assessment from which you wish to be exempt. The form is to be completed each year by the parent in order for the student to be exempt in the current school year. The forms will also be available in the school office.
